HLA-B27, Blood
Use
This test is used to assist in the diagnostic process of ankylosing spondylitis, juvenile rheumatoid arthritis, and reactive arthritis. The presence of the human leukocyte antigen (HLA)-B27 is associated with these conditions, though the exact mechanism of the association is not understood and is thought to be due to linkage disequilibrium. HLA-B27 is present in a significant portion of patients with these diseases.
Special Instructions
This test is best used alone if a particular disease, such as ankylosing spondylitis, is under consideration. Orders for both this test and other HLA tests (1LRR or 1DIS) will be questioned due to overlap.
Limitations
HLA-B27 testing is not diagnostic on its own and should be interpreted in the context of clinical findings and other diagnostic procedures. The test may give false negatives or positives in individuals of certain ethnic backgrounds, especially if the HLA-B*27:06 allele is present, which is more common in those of Asian ethnicity and not associated with ankylosing spondylitis.
