MAML2 (11q21) Rearrangement, Mucoepidermoid Carcinoma (MEC), FISH, Tissue
Use
This test is useful for identifying MAML2 rearrangements, which are detectable in 80% to 85% of mucoepidermoid carcinomas. It supports the diagnosis of mucoepidermoid carcinoma when used in conjunction with an anatomic pathology consultation. MAML2 rearrangements are not found in morphologic mimics but can be identified in other neoplasms such as hidradenoma, poroma, porocarcinoma, and hemangioendothelioma.
Special Instructions
A pathology report is required for testing to be performed. Specimens submitted must include a patient name, block number, date of collection, and tissue source. A reason for testing must be provided; otherwise, Mayo Clinic Laboratories may enter an appropriate indication for testing. The test does not include a pathology consultation; if needed, PATHC / Pathology Consultation should be ordered.
Limitations
This test is not cleared or approved by the US Food and Drug Administration and is best used as an adjunct to existing clinical and pathologic information. The fluorescence in situ hybridization (FISH) assay does not rule out other chromosome abnormalities. Non-formalin fixed specimens may not succeed fully in FISH assays. Decalcified paraffin-embedded specimens have about a 50% success rate for FISH analysis. FISH studies require sufficient tumor tissue for analysis, and lack of sufficient tissue post-hybridization will result in a lack of FISH results.

Collection Instructions
Submit a formalin-fixed, paraffin-embedded tumor tissue block. Blocks with alternative fixation methods like Prefer or Bouin's are less likely to succeed. Paraffin-embedded specimens can be from any anatomic location. Fresh tissue will be canceled upon receipt. If only fresh tissue is available, embed in paraffin before sending.
