Monoclonal Protein Screen, 24 Hour, Urine
Use
Monitoring patients with monoclonal gammopathies; detection of urine monoclonal proteins (M‑proteins) including identification of immunoglobulin light chains as seen in conditions such as multiple myeloma, primary systemic amyloidosis, light‑chain deposition disease, monoclonal gammopathy of undetermined significance, and idiopathic Bence‑Jones proteinuria. Urine total protein concentration, electrophoretic patterns, and M‑protein isotype are reported. Protein fractions (albumin; alpha‑1, alpha‑2, beta, gamma globulins) are quantified, and an M‑protein “M‑spike” may be identified.
Special Instructions
This profile includes M‑protein Mass‑Fix (MALDI‑TOF MS) and total protein (turbidimetry); if MASSFIX is positive for M‑protein, protein electrophoresis is reflexively performed at additional charge. Requisition via Renal Diagnostics Test Request (T830) if not ordered electronically. Collect 24‑hour urine. Aliquot 30–50 mL into plastic 60 mL urine bottle. Add preservatives or apply temperature controls within 4 hours of collection. Forms and instructions available (e.g., Urine Preservatives‑Collection and Transportation for 24‑Hour Urine Specimens). Send refrigerated (preferred); plastic urine container, 60 mL (T313). Record volume (mL) and collection duration.
Limitations
MASSFIX alone is not considered adequate screening; must reflex to electrophoresis if positive. Hemolysis, radiographic agents, or penicillin may produce interference or false patterns (e.g., penicillin splits albumin band). Monoclonal gammopathies rarely seen in patients under 30. Interpretation depends on proper collection (24‑hour duration, adequate volume). Total protein reference value established only for age 18+, fractions reported when present.
