Multiple Myeloma Measurable Residual Disease (MRD), Flow Cytometry, Bone Marrow
Use
Detecting low-level measurable residual disease (MRD) in patients with multiple myeloma post-therapy is a significant prognostic marker. The presence of MRD indicates remaining cancerous plasma cells, which can predict patient outcomes. This test employs high-sensitivity flow cytometry, following EuroFlow guidelines, to assess MRD levels with a sensitivity of 10(-5) or better. It is essential for monitoring remission and guiding treatment decisions post-chemotherapy or autologous stem cell transplantation in multiple myeloma patients.
Special Instructions
This test should be ordered on patients treated for multiple myeloma to confirm remission status. It is not suitable for patients with known relapse or at diagnosis. Specimens should arrive within 2 days of collection. Include patient’s disease state (e.g., untreated, treated, stable) and available Immunofix information.
Limitations
Current gating strategies may not detect abnormal plasma cells if they do not phenotypically differ from normal cells. Therapeutic antibodies may also interfere with antigen expression. Specimen processing affects plasma cell percentage, and sensitivity may diminish with low event collections.
