Phospholipid (Cardiolipin) Antibodies, IgA, Serum
Use
This test may be of diagnostic significance for patients at risk for antiphospholipid syndrome (APS) or systemic lupus erythematosus (SLE) who test negative for criterial antiphospholipid antibodies. APS is characterized by thrombosis and/or pregnancy-related morbidities associated with persistent positivities of aPL antibodies. It is crucial for determining APS and often tested alongside other aPL antibody tests, such as lupus anticoagulant (LAC) and IgG/IgM to cardiolipin and anti-beta2-glycoprotein I. Testing is recommended for noncriteria aPL antibody tests in cases with high disease suspicion but inconclusive results using other criteria.
Special Instructions
Samples should be collected, centrifuged, and aliquoted into a plastic vial as per collection instructions. Ensure the correct procedure to avoid specimen rejection due to gross hemolysis, lipemia, or heat treatment. The test involves the use of solid-phase enzyme-linked immunoassay and other relevant immunoassays to detect aCL and anti-B2GPI antibodies, with appropriate antibody cutoff values for positivity assessment.
Limitations
Immunoassays used may not completely distinguish between autoantibodies for APS and those from infectious responses. Evaluation of results may vary with different commercial assays. Interpretation should consider persistence based on Sapporo criteria. Measurement of aCL and anti-B2GPI IgA lacks formal guidance, and clinical relevance of isolated IgA is minimal. Transient antibody production and variability between assay types should be accounted for in clinical practice.
