Clinical Use

Use

This test is used for the assessment of T-cell receptor diversity in various clinical contexts, including inborn errors of immunity (formerly primary immunodeficiencies), monitoring immune reconstitution post-hematopoietic stem cell transplantation, and assessing repertoire changes in autoimmune diseases and viral infections. It provides a global view of TCR repertoire diversity, which is useful for most clinical applications where this level of assessment is required.

Special Instructions

Approval from Mayo Clinic Laboratory Director/Consultant is required for patients over 40. Specimen collection should be synced with shipping time to ensure arrival within 24 hours.

Limitations

This is a qualitative/semiquantitative assay and does not quantify the amount of transcript for each T-cell receptor. It is not a deep sequencing assay and doesn't provide the granularity of information offered by deep sequencing. Not validated for determining clonality in hematologic malignancies. Should not be ordered for low CD3+ T-cell count (<70 cells/mcL) or without prior discussion for patients over 40.
