Use
This test is useful for individuals with IgA deficiency, evaluating patients suspected of having celiac disease (including those with compatible clinical symptoms, atypical symptoms, or an increased risk due to family history, previous diagnosis with associated disorder, or positivity for HLA DQ2 and/or DQ8), screening for dermatitis herpetiformis in conjunction with an endomysial antibody test, and monitoring response to a gluten-free diet in patients with dermatitis herpetiformis and celiac disease.
Special Instructions
Cascade testing is recommended for celiac disease to proceed in an algorithmic fashion. Some recommended cascades include Celiac Disease Comprehensive Cascade, Celiac Disease Serology Cascade, and Celiac Disease Gluten-Free Cascade. In cases of electronic order restrictions, complete, print, and send a Gastroenterology and Hepatology Test Request (T728) with the specimen.
Limitations
This test should not solely establish the diagnosis of celiac disease. It is primarily useful for identifying patients with an increased probability, in whom an intestinal biopsy may be warranted. Negative results could necessitate further investigation with endoscopy and biopsy, especially for patients with malabsorptive symptoms. Testing should be performed while the patient is on a gluten-containing diet.
