Drug Toxicology Amphetamines, with Confirmation, Oral Fluid
Use
This test detects the presence of amphetamines in oral fluid, serving as an alternative to urine testing. Oral fluid testing can detect recent drug use more rapidly due to drug entry into salivary glands sooner after ingestion, and is especially useful where observed collection is needed to reduce tampering risks. If initial screening results meet or exceed cutoff thresholds, confirmation testing is performed for definitive identification. This offers a reliable method to evaluate recent amphetamine ingestion in clinical and forensic contexts.
Special Instructions
Specimens must be collected using the Quantisal™ oral fluid collection device, which collects 1 mL of oral fluid into a 3 mL buffer tube. Donors should avoid food, drink, gum, or tobacco for 10 minutes prior to collection; if the mouth is not empty, rinse with up to 4 oz water and wait at least 10 minutes prior to collection. Use of non‑Quantisal devices, food debris, multiple or missing swabs, or inverted wand presence are criteria for rejection. Confirmation testing incurs additional charge and CPT codes apply. The test is performed at Quest Diagnostics Nichols Institute, Chantilly, VA. Order CPT codes for confirmation include AMPhetamines – 80324 (HCPCS: G0480 or G0481).
Limitations
Oral fluid has a shorter detection window (often 24–50 hours) than urine, and drug concentrations may be lower. Detection does not indicate time of dose or amount ingested, and positive results require confirmation via GC/MS or LC‑MS/MS. Quantitative results should be interpreted cautiously and are not reliable for estimating dose. Oral fluid may contain variable components such as saliva, cellular debris, and gingival fluid, potentially affecting detection.
