This age‑based “Smart Code” test for cervical cancer screening integrates image‑guided Pap cytology with reflex testing for high‑risk HPV mRNA and screening for chlamydia and gonorrhea depending on age-defined criteria. It aligns with ACOG, USPSTF, and other guideline recommendations for women 21–65 to optimize detection of cervical precancer and associated infections using a combined screening approach. The test adapts screening components according to patient age and cytology outcomes, offering efficient, tailored, guideline‑based care.
Special Instructions
Smart Code simplifies ordering by combining multiple components—including image‑guided Pap (ThinPrep® or SurePath™), HPV mRNA reflex testing, and CT/NG testing where indicated—based on patient age and cytology results. The panels’ components can also be ordered separately if needed. Ordering follows ACOG age‑based guidelines; for more detail, individual components or test codes may vary by location.
Limitations
Performing Pap alone without associated reflex or infection testing may miss high‑risk HPV infections or coexisting sexually transmitted infections. Age-based logic depends on accurate age documentation and cytology result interpretation (e.g., ASC‑US). The reflex HPV and genotyping are contingent on Pap results and HPV positivity, respectively, which could delay full diagnostic information. Reflex tests incur additional charges and may involve non‑orderable reflex codes.
