Beta‑2 Microglobulin, Random Urine with Creatinine
Use
Measurement of urinary beta‑2 microglobulin normalized to creatinine helps assess renal tubular dysfunction and proximal tubular injury. It’s also used to monitor exposure to heavy metals such as cadmium and mercury given increased excretion in such settings.
Special Instructions
This test uses a Quest Diagnostics Beta‑2 Microglobulin Transport Tube (available through Client Supply) to collect exactly 10 mL random urine. If the tube isn’t available, urine must be pH‑adjusted to 6–8 using 1 N NaOH prior to shipping because beta‑2 microglobulin is unstable in acidic urine (pH <6).
Limitations
Beta‑2 microglobulin is unstable in acidic urine (pH <6); sample volume and pH control are critical. Collection outside instructed volume or failure to use proper transport tube (or pH adjustment) may lead to degradation and invalid results. Test intended for assessment of tubular function and heavy‑metal exposure, not for glomerular diseases alone.

Collection Instructions
Patient should void bladder, drink at least 500 mL (approximately 17 oz) water, collect urine within 1 hour, pour exactly 10 mL into transport tube. If tube unavailable, adjust pH to 6–8 with 1 N NaOH.
