Use
Appropriate test to rule out acute intermittent porphyria (AIP) and other acute attack types of porphyrias associated with neurologic and/or psychiatric symptoms. Detect porphyrias and porphyrinurias. Acute intermittent porphyria is characterized by excretion of porphobilinogen and delta aminolevulinic acid in urine during acute attacks, but not during latency. Increased urine values are seen in the intoxication porphyrias (heavy metals, halogenated solvents, certain drugs).
Special Instructions
Record total 24-hour urine volume on the test request form. If ordering random urine testing, use Porphobilinogen (PBG), Quantitative, Random Urine [003065].
Limitations
The test was developed and its performance characteristics were determined by Labcorp. It has not been cleared or approved by the Food and Drug Administration. It may fail to detect some latent cases of acute intermittent porphyria, as not all patients show elevated porphobilinogen levels during non-attack periods.

Collection Instructions
Instruct the patient to void at 8 AM (or 8 PM) and discard the specimen. Then collect all urine including the final specimen voided at the end of the 24-hour collection period in the container. Keep refrigerated during collection. Measure and record the total volume on the request form. Mix well before transferring to an amber transport tube. Freeze immediately.
Storage Instructions
Freeze immediately and protect from light. Stable for one month when preserved with acetic acid and frozen, or for 24 hours refrigerated.
