Porphyrins, Fractionation and Quantitation, Urine

Use
This test is used to evaluate cutaneous photosensitivity and to exclude or confirm the diagnosis of porphyria cutanea tarda (PCT). Elevated concentrations of urine uroporphyrin and heptacarboxyl, hexacarboxyl, and pentacarboxyl porphyrins suggest porphyria cutanea tarda, typically associated with cutaneous photosensitivity. Additionally, this test includes creatinine normalization to aid in interpretation, especially beneficial for samples with variable collection times and volumes.
Special Instructions
Collect the urine specimen over 24 hours or as a random sample. It must be refrigerated during collection and protected from light. Record total volume and collection time interval on the ARUP Amber Transport Tube and test request form.
Limitations
Results are normalized to creatinine concentration and expressed as a ratio of micromoles of porphyrin per mole of creatinine. Interpretation should consider deviations in collection time or urine volume. For neurologic and psychiatric symptom evaluation related to acute porphyrias, urine porphobilinogen testing is required.
