Use
The test is used as an initial screening for syphilis under the traditional algorithm and may also be used to confirm a reactive treponemal test in the reverse algorithm. It is a suitable test for monitoring treatment response in established syphilis cases. The VDRL test, while a common screening tool, is less sensitive than the RPR test, thus it may miss some infections.
Special Instructions
If the VDRL test is reactive, a titer will be added to determine the concentration of antibodies. This will incur additional charges. It is important to transfer serum from cells as soon as possible or within 2 hours of collection for optimal results.
Limitations
The VDRL test is less sensitive than the Rapid Plasma Reagin (RPR) test when used as a screening method for syphilis. This reduced sensitivity can lead to missed infections if this is the sole testing method. Additionally, the test is reactive to conditions causing similar non-specific antibodies, potentially causing false positives. Performance may also vary due to specimen handling, so conditions like contamination, hemolysis, or severe lipemia can affect results.
