BCL6 (3q27) Gene Rearrangement by FISH
Use
This test is used for detecting BCL6 gene rearrangements in formalin-fixed, paraffin-embedded (FFPE) tissues, particularly in cases of B-cell lymphomas such as diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma. The presence of BCL6 rearrangements can be crucial for prognosis and diagnosis in these lymphomas. It helps in correlating clinical, morphological, and immunophenotypic data to improve patient management.
Special Instructions
Include the surgical pathology report with the specimen submission. If submitting multiple specimens, include an order comment specifying specimen selection by pathologist or submit individual orders for each sample. A pathologist block selection fee is applicable if asking ARUP to choose the specimen for testing.
Limitations
This FISH test is designed to detect translocations involving the 3q27 locus (BCL6) without regard to partner chromosomes. It is sensitive to BCL6 rearrangements, except in rare cryptic rearrangements. Results should be interpreted within the context of clinical and pathological information. The test’s performance was validated with known positive and normal cases, but it is not cleared by the FDA and should be used for clinical purposes only.

Collection Instructions
Formalin fix (10 percent neutral buffered formalin) and paraffin embed specimen. Transport tissue block or 5 unstained (3-micron thick sections), positively charged slides. Do not oven bake pre-cut slides.
Storage Instructions
Protect paraffin block and/or slides from excessive heat. Ship in cooled container during summer months.
Causes for Rejection
Paraffin block with no tumor tissue remaining. Specimens fixed in any fixative other than 10 percent neutral buffered formalin. Decalcified specimens.
