Special Instructions
A separate specimen is required for culture. Ensure proper specimen collection and transport to maintain sample integrity. The test is ordered to confirm the presence of Toxin B in patients suspected of having Clostridioides difficile infection, often following initial testing by other methods such as EIA or PCR.
Limitations
This assay is specific for the detection of Toxin B only and does not detect Toxin A or other virulence factors of Clostridioides difficile. False negatives may occur if the specimen is not collected or transported under optimal conditions, as the toxin is labile and might degrade. Additionally, a negative result does not rule out C. difficile infection as lower levels of toxin or non-toxigenic strains may not be detected.

Storage Instructions
Maintain specimen at 2°C to 8°C and transport to the laboratory within 24 hours of collection. If a longer period is required, the specimen should be frozen at –70°C or maintained on dry ice.
Causes for Rejection
Inappropriate specimen transport conditions (eg, room temperature) or device; improper labeling; specimen received after prolonged delay (usually more than 72 hours); leaking specimen; specimen received in denture cup, “Cool Whip” container, margarine container, or similar container
