FISH, Multiple Myeloma, IGH/FGFR3, t(4;14)
Use
This FISH assay targets chromosomal translocations associated with multiple myeloma, specifically the IGH/FGFR3 t(4;14) rearrangement as part of disease prognostic evaluation. It is designed to detect cytogenetic abnormalities that inform risk stratification in new diagnoses of plasma cell myeloma and guide initial therapeutic planning.
Special Instructions
Reflex testing may be performed following initial detection of IGH rearrangement according to Quest Diagnostics protocol. Specimen should be bone marrow aspirate with ≥10% plasma cells; CD138 enrichment may be performed if sample volume permits. Peripheral blood and core biopsies are not acceptable.
Limitations
Specimen viability may decline rapidly post‑aspiration, compromising enrichment or culture. Plasma cell culture may take up to 5 days before harvest, delaying turnaround. Limited aspirate volume or low plasma cell percentage may require partial panel cancellation, with high‑risk probes prioritized.
