Autoimmune Epilepsy Panel, CSF

Use
This test is used to evaluate new acute-to-subacute onset epilepsy and incomplete seizure control of less than 2 years. It is part of a diagnostic approach for autoimmune neurological diseases, assessing antibodies that may be associated with autoimmune epilepsy. Testing is performed in both cerebrospinal fluid (CSF) and serum to maximize diagnostic yield.
Special Instructions
This panel is recommended for evaluating neurological symptoms indicative of autoimmune epilepsy. It includes management pathway recommendations and requires specific specimen transportation and stability conditions. If LGI1, CASPR2, AMPA, GABA-BR, DPPX, GABA-AR, mGluR1, NMDA, or CV2 antibodies are positive, additional titers will be performed with additional charges. New York state approved. Freeze CSF specimens immediately after collection and transport them frozen; avoid repeated freeze/thaw cycles.
Limitations
This test might not identify all cases of autoimmune epilepsy since antibody presence can be intermittent or absent in some patients with this condition. Negative results do not rule out autoimmune encephalitis. Clinical correlation is crucial, and further diagnostic evaluation may be necessary in the context of persistent or atypical clinical presentations. The test has not been cleared or approved by the US FDA but is performed in a CLIA-certified laboratory for clinical purposes.
