Flunitrazepam and Metabolites, Serum or Plasma, Screen with Reflex to Confirmation/Quantitation

Use
This test is used to detect the exposure to Flunitrazepam, commonly known by its brand name Rohypnol, and its metabolites in serum or plasma. Detection is performed using qualitative high-performance liquid chromatography-tandem mass spectrometry. It supports clinical toxicology, particularly in cases requiring emergency toxicology assessment for benzodiazepines. The reflex to confirmation and quantitation allows for accurate confirmation and measurement of detected substances when the initial screen is positive.
Special Instructions
Specimen must be separated from cells as soon as possible, ideally within 2 hours of collection. Only plasma or serum collected in plain red, lavender (EDTA), or pink (K2EDTA) tubes is accepted. Avoid separator tubes as they are unacceptable. The test has a reflex confirmation process, and additional charges may apply for the confirmation test.
Limitations
The test is not performed at ARUP Laboratories but by a contracted laboratory, which may result in variations in testing protocols and detection limits. The effectiveness of the test in detecting Flunitrazepam and its metabolites can be influenced by the time elapsed post-ingestion and the dosage. Detection limits are set at 2.0 ng/mL, and results below these levels will not be reported. Cross-reactivity with other substances and the possibility of false positives/negatives should be considered when interpreting results.
