Use
This test is useful in assessing both acute and chronic exposure to arsenic, mercury, and lead. It is particularly beneficial for monitoring occupational exposure to these heavy metals. The urinary concentrations can indicate the level of exposure to these metals, and help in monitoring chelation therapy. For specimens with elevated total arsenic results, an automatic reflex is performed to determine proportions of inorganic, methylated, and organic species.
Special Instructions
Diet, medication, and nutritional supplements may introduce interfering substances, thus patients should stop these, and avoid shellfish and seafood for 48 to 72 hours prior to specimen collection. Specimen collection is not recommended within 72 hours of iodinated or gadolinium-based contrast media administration due to potential interference.
Limitations
Elevated results might be due to contamination from the skin or improper collection techniques, such as using non-trace element-free containers. Confirmation with another certified trace element-free container may be needed. In the case of arsenic, only the inorganic species are considered most toxic; thus, fractionation helps in assessing the true risk.

Patient Preparation
Discontinue nutritional supplements, vitamins, minerals, nonessential over-the-counter medications upon physician advice, and avoid shellfish and seafood for 48 to 72 hours.
Storage Instructions
Refrigerated; also acceptable at Room temperature or frozen.
Causes for Rejection
Urine collected within 72 hours after administration of iodinated or gadolinium-based contrast media. Acid-preserved urine. Specimens contaminated with blood or fecal material.
