Creatine and Guanidinoacetate Determination - Urine
Use
This test provides quantitative analysis of creatine and guanidinoacetate in urine, which is useful for evaluating patients suspected of having inborn errors of creatine metabolism, such as Guanidinoacetate Methyltransferase deficiency (GAMT deficiency), Arginine:Glycine Amidinotransferase deficiency (AGAT deficiency), and Creatine Transporter deficiency (CTD). Creatine deficiency syndromes often lead to intellectual disabilities, seizures, and disturbances in cognitive speech. In cases of AGAT deficiency, there are typically low levels of guanidinoacetate and creatine in the tested samples, whereas GAMT deficiency shows increased levels of guanidinoacetate. Males with CTD present an increased creatine-to-creatinine ratio. Thus, this test aids in diagnosing these metabolic disorders by analyzing urine samples for the presence of specific metabolites.
Special Instructions
We recommend that Plasma Creatine and Guanidinoacetate Determination (test code 4130) is ordered concurrently with this test. Indications for Testing include patients with a history of seizures, global developmental delay, failure to thrive, speech delay, and/or behavioral difficulties.
Limitations
False positives or negatives may occur if the patient does not properly collect the urine sample or if the specimen is not stored at the correct temperature. Environmental factors, medications, and other physiological conditions may affect metabolite levels. This test should not be used solely for diagnosis; clinical correlation is necessary. Additionally, certain variants of the associated conditions may not be detected by this test.
