Measurement of Aspergillus fumigatus IgG levels
Special Instructions
The test was developed by Labcorp and has not been cleared or approved by the FDA. It is important to consider the clinical context and radiological evidence in conjunction with serological findings when diagnosing and managing aspergillosis-related conditions. In cases where patients have active chronic lung disease, measurement of Aspergillus IgG may not be sufficient for diagnosing CPA, and further clinical and radiological evidence is required.
Limitations
The test can face limitations as the cutoffs for diagnosing ABPA are similar to those used for CPA, and there is no value of A. fumigatus IgG that can discriminate strongly between these disorders. Aspergillus IgG may remain negative in some patients with symptoms, and measurable IgG levels can be present in patients with lung disease without CPA. The assay may not detect IgG to non-fumigatus strains in populations where these are significant. Additionally, the test is not useful for diagnosing invasive pulmonary aspergillosis (IPA) in severely immunocompromised patients due to a lack of antibody production.
