Use
The Autoimmune Liver Disease Reflexive Panel is designed as a recommended initial screening panel when autoimmune liver disease is strongly suspected. It aims to identify specific autoantibodies associated with conditions such as primary biliary cholangitis, autoimmune hepatitis, and primary sclerosing cholangitis. The presence of these antibodies helps differentiate between these diseases, guiding further diagnostic and therapeutic decisions. In cases where the panel results are negative, additional less common autoantibody tests may be ordered based on the patient's clinical presentation and history to ensure comprehensive evaluation.
Special Instructions
Samples should be collected using a serum separator tube (SST) immediately separated from cells within two hours of collection to ensure sample integrity. Transfer a minimum of 1.0 mL serum to an ARUP standard transport tube for analysis. Specimens must be refrigerated to maintain stability and prevent degradation. Grossly hemolyzed, lipemic, or icteric samples, and those with fibrin clots, are considered unacceptable for analysis.
Limitations
This panel does not rule out autoimmune liver diseases if the test results are negative, as antibodies may not be present or detectable in all cases. F-Actin antibodies, while indicative, have lower specificity compared to Smooth Muscle antibodies. Similarly, the presence of SLA antibodies, though highly specific for autoimmune hepatitis, is only found in a subset of patients. Therefore, negative results should be interpreted with caution and in the context of other clinical findings. Additional specific serologic tests may be necessary for confirmation.
