Drug-Induced Liver Injury Profile
Use
This test is utilized for the initial laboratory assessment of liver injury caused by drugs, herbal products, and dietary supplements. It helps in identifying significant biochemical evidence of drug-induced liver injury, such as elevated serum aminotransferase levels (AST or ALT), alkaline phosphatase (ALP), or bilirubin levels, along with international normalized ratio (INR) evaluations. When suspected liver injury falls into hepatocellular or mixed categories, this test aids in excluding other potential causes of liver injury.
Special Instructions
Ensure proper specimen handling according to specified guidelines. Serum or plasma should be separated from cells within 45 minutes of collection. Blood for PT with INR should be collected in a blue-top tube containing 3.2% buffered sodium citrate. Immediate mixing of the blood with the anticoagulant by gently inverting at least six times is necessary.
Limitations
The Prothrombin Time (PT) test may not detect slight deficiencies in single clotting factors and could be affected by the presence of lupus anticoagulants or anticoagulant therapy (e.g., heparin, hirudin, argatroban). Alterations in PT results may not reliably reflect the level of anticoagulation in patients on certain anticoagulants, thus a chromogenic factor X assay might be indicated for monitoring. Heparin and other such anticoagulants may extend PT, affecting the interpretations.
