Hemoglobin Evaluation by HPLC With Reflex to Electrophoresis and/or RBC Solubility

Use
The Hemoglobin Evaluation by HPLC with Reflex to Electrophoresis and/or RBC Solubility is an effective test for screening hemoglobinopathies, which may include conditions such as thalassemias and unstable hemoglobins. It is also suitable for follow-up in individuals with a known hemoglobinopathy. This screening process aids in identifying variants that cannot be quantitated directly by HPLC, with reflex testing to RBC Solubility or Capillary Electrophoresis for confirmation and identification of the variant.
Special Instructions
If abnormal peaks suggestive of a hemoglobin variant are detected, reflex tests including RBC Solubility and/or Capillary Electrophoresis will be performed. Additional charges may apply for these reflex tests. The quantitation of hemoglobin is recommended for definitive diagnoses, particularly in infants aged one year or older.
Limitations
The test may not detect all possible hemoglobin variants. Normal results from HPLC and capillary electrophoresis do not rule out the possibility of alpha globin gene deletions associated with silent carrier status or alpha thalassemia trait. Additionally, individuals with certain rare thalassemia variants may not be identified by this assay if they exhibit normal Hb A2 levels.
