Hemoglobin S Evaluation with Reflex to RBC Solubility

Use
This test is used to determine the presence of hemoglobin S, and is recommended for detecting sickle cell trait in athletes. The test uses High Performance Liquid Chromatography (HPLC) to identify the presence of hemoglobin S, and if detected, it is confirmed with RBC solubility testing. The test helps differentiate hemoglobin S from other abnormal hemoglobin variants.
Special Instructions
If HPLC detects a peak suggesting hemoglobin S, RBC solubility testing is added for confirmation, which incurs additional charges. This test is New York state approved and can be ordered using ARUP test code 3016616. For further clarification on differentiating hemoglobin S trait from related conditions, further evaluation using ARUP test 3017101 is recommended.
Limitations
This test does not differentiate hemoglobin S trait from homozygous sickle cell disease or other combinations such as S/C, S/D, S/G, S/E, S/thalassemia, S/O-Arab, S/New York, and C-Georgetown trait (Hb C-Harlem). For a more detailed evaluation, additional testing methods like electrophoresis might be necessary.
