HLA Class I Antibody, Panel-reactive Antibody (PRA)
Also known as: HLA Antibody, PRA, HLA Class 1 Antibody, White Cell Antibody
Use
Solid organ transplantation
Special Instructions
Collect specimen prior to dialysis. Avoid collection if the patient has recently received antithymocyte globulin, OKT3, or alemtuzumab (Campath®) as these drugs may cause false-positive results. To prevent delays in turnaround time for frozen samples when multiple tests are requested, submit separate frozen specimens for each test. Maintain the serum specimen at room temperature and protect it from extreme heat or cold, although it may also be frozen if necessary.
Limitations
The test detects antibodies of the IgG isotype; it does not detect IgM antibodies, which are often indicative of autoantibodies. This limitation means that the test focuses solely on IgG-mediated immune responses and may not fully characterize the antibody profile related to potential graft rejection. Furthermore, factors such as lipemic or hemolyzed specimens can cause interference, potentially leading to rejection of the sample. Despite these limitations, the test offers valuable insights into compatibility for organ transplantation.
