Use

Aid in the diagnosis of aggressive large B-cell lymphoma with intermediate features between Burkitt lymphoma and diffuse large B-cell lymphoma (DLBCL) and confirmation of suspected double-hit lymphoma. The test can detect 8q24 (MYC) translocations and is performed using FISH to assess MYC rearrangement in various B-cell lymphomas, including 'double-hit' or 'triple-hit' lymphomas. The results should be correlated with clinical, morphologic, and immunophenotypic data.

Special Instructions

Include surgical pathology report. If multiple specimens (blocks or slides) are sent to ARUP, they must be accompanied by an order comment indicating that the ARUP pathologist should choose the specimen most appropriate for testing or individual orders for each specimen submitted. A Pathologist Block Selection Fee (ARUP test code 3002076) will be added to orders that utilize the first option.

Limitations

The test is expected to detect 100 percent of MYC rearrangements in patients with MYC-rearranged lymphomas, except for rare instances of cryptic rearrangements. The use of this assay on decalcified tissues has not been validated. Results should be interpreted with caution. Some signal patterns other than the classic abnormal pattern may also be present and may be considered abnormal.

Collection Instructions

Formalin fix (10 percent neutral buffered formalin) and paraffin-embed tissue. Transport tissue block or slides in a tissue transport kit.

Storage Instructions

Room temperature. Also acceptable: Refrigerated. Protect paraffin block from excessive heat. Ship in cooled container during summer months.

Causes for Rejection

Paraffin block with no tumor tissue remaining. Specimens fixed in any fixative other than 10 percent neutral buffered formalin. Decalcified specimens.
