Use
The Cadmium Exposure Panel is recommended for occupational exposure monitoring and includes tests for cadmium in blood and urine as well as beta-2-microglobulin. These tests provide guidance on cadmium exposure in accordance with OSHA regulations. Blood cadmium levels are used for monitoring acute toxicity, while urine cadmium levels assess body burden. Beta-2 microglobulin in urine serves as an early marker of irreversible kidney damage, which can occur due to cadmium exposure.
Special Instructions
To avoid contamination, specimens should be collected at the beginning of a work shift. It is recommended to avoid collection within 72 hours of administration of iodinated or gadolinium-based contrast media. Patients should be encouraged to discontinue nonessential over-the-counter medications and supplements to avoid interference.
Limitations
Elevated blood cadmium levels might be due to skin or collection-related contamination, and if contamination is suspected, a second specimen should be collected using certified trace element-free tubes. Urine beta-2 microglobulin is sensitive to very dilute urines, with a creatinine concentration below 20 mg/dL, which would require repeat collection. Interfering substances from diet, medication, or supplements can affect urinary analyses, and high iodine concentration might lead to inaccurate elemental testing results.
