Use
The Lactate Dehydrogenase, Isoenzymes test is used to evaluate elevated lactate dehydrogenase associated with noncardiac muscle injury. It is not recommended for detecting myocardial injury. In clinical practice, troponin testing is often preferred for the diagnosis and management of acute coronary syndrome. This test can help differentiate between various causes of elevated lactate dehydrogenase levels in the blood by identifying which isoenzymes are elevated.
Special Instructions
The test should not be used to detect myocardial injury; instead, troponin testing is recommended for acute coronary syndrome. It is crucial to separate serum from cells as soon as possible, preferably within 2 hours of collection, to avoid erroneous results. Hemolyzed specimens and specimens not maintained at recommended temperatures can affect the levels of LD isoenzymes.
Limitations
The levels of LD-1 and LD-2 isoenzymes may be elevated in hemolyzed specimens and serum not promptly separated from cells, potentially leading to misinterpretation. Additionally, LD-3, LD-4, and LD-5 isoenzymes are sensitive to low temperatures and may show erroneously low levels if the specimen has been refrigerated or frozen. It is essential to adhere to specimen handling and temperature instructions to ensure accurate results.
