Special Instructions
For successful isolation of Legionella species, it is crucial to avoid contamination with normal flora from the respiratory tract. Only suitable specimens such as transtracheal aspirates, pleural fluid, or other body tissues offer optimal results for culture. Specimens must be transported to the laboratory within 24 hours of collection to maintain viability. Healthcare providers should contact the laboratory for assistance regarding specimen collection and handling.
Limitations
This culture test's sensitivity ranges from 50% to 80%, though its specificity is 100%. Sputum and bronchial aspirates may suffer from bacterial overgrowth, decreasing the test's effectiveness. A direct fluorescent antibody smear without culture can alternatively detect Legionella, and acute and convalescent sera for Legionella antibodies might be required to maximize diagnostic accuracy. However, seroconversion, a critical diagnostic criterion, may take time to manifest, with only 71% detection at 28 days post-onset. Environmental factors in hospitals can also influence transmission.
