Tobramycin, Peak and Trough

Use
This test measures serum concentrations of the aminoglycoside antibiotic tobramycin at both peak and trough levels. Tobramycin monitoring helps ensure therapeutic efficacy and minimize toxicities such as nephrotoxicity and ototoxicity, particularly in patients being treated for serious gram‑negative infections.
Special Instructions
Peak levels are typically drawn shortly after infusion completion; trough levels are drawn immediately prior to the next dose. Use of appropriate serum separator or gel‑free tubes and collection timing are critical for accurate measurement.
Limitations
Levels can be affected by improper timing of specimen collection, use of gel separator tubes which may adsorb drug, and delays in processing. Interpretation must consider timing relative to dosing and patient renal function; mis-timed or hemolyzed specimens may yield misleading results.
