Urticaria-Inducing Activity with Thyroid Antibodies and Stimulating Hormone

Use
Chronic urticaria (CU) is a common and complex dermatological condition characterized by persistent hives lasting for more than six weeks. This condition is often suspected in patients who do not exhibit symptoms of an exogenous allergen. Approximately 45 percent of CU cases involve autoantibodies that target basophils or mast cell-associated IgE receptors, such as the high-affinity IgE-Fc receptor (Fc epsilon R1 alpha). Evaluating the presence of histamine-releasing factors in patient serum can provide insights into the basophil and mast cell activation status, which can aid in the diagnosis of CU.
Special Instructions
Patients taking calcineurin inhibitors should discontinue their medication 72 hours before the blood draw. Those on prednisone should be off their medication for two weeks prior to the draw. ARUP Laboratories developed this test, which has not been cleared or approved by the US Food and Drug Administration.
Limitations
This panel includes tests for thyroid peroxidase (TPO) antibody, thyroglobulin antibody, thyroid stimulating hormone, and urticaria-inducing activity. Each component provides specific insights into the presence of autoantibodies and the stimulation of basophils, which may not represent comprehensive results for all cases of chronic urticaria. Factors such as sample contamination, hemolysis, or lipemia may affect results.

Collection Instructions
Separate serum from cells ASAP or within 2 hours of collection, transfer 1 mL serum to two ARUP standard transport tubes, and freeze immediately.

Patients should stop calcineurin inhibitors 72 hours before and prednisone two weeks before draw.
