Amikacin, Peak

Use
Amikacin is used to treat serious infections caused by susceptible aerobic gram-negative bacilli (eg, a number of the Enterobacteriaceae, P aeruginosa). Such infections include bacteremias; intra-abdominal, soft tissue (including burns), bone or joint, lower respiratory tract, and complicated urinary tract infections, and meningitis.
Special Instructions
Peak levels should be ordered with Test No. 007204, while trough levels should be ordered with Test No. 007205. Tubes must be labeled as 'peak' or 'trough'. Creatinine levels should be monitored every 2-3 days to indicate potential renal toxicity. Amikacin's nephrotoxicity is linked to prolonged trough levels >8 μg/mL. Oliguric renal failure might result from continued administration but is mostly reversible upon discontinuation.
Limitations
Amikacin therapy may result in nephrotoxicity in 10-25% of patients receiving aminoglycosides, predominantly due to prolonged high trough levels. Monitoring of plasma levels is essential to avoid this risk. Aminoglycosides may also cause ototoxicity, although it's uncommon when levels are closely monitored and dosing is adjusted. High peak serum concentrations from new dosing regimens do not necessarily enhance toxicity, complicating therapeutic monitoring.

Collection Instructions
Transfer separated serum or plasma to a plastic transport tube. Peak samples are drawn 60 minutes after an IM injection, 30 minutes after a 30-minute IV infusion, or immediately after a 60-minute IV infusion.
