Lupus Panel 3 with ANA, IFA with Reflex to Titer and Pattern
Also known as: Systemic Lupus Erythematosus (SLE), FANA, Antinuclear Antibody Screen, Fluorescent ANA, Hep-2
Use
This panel can be helpful in the diagnosis and management of systemic lupus erythematosus (SLE). The panel simultaneously tests for antinuclear antibodies (ANAs) using an immunofluorescence assay (IFA) on HEp‑2 cells, as well as six specific autoantibodies associated with SLE and three biomarkers associated with disease activity. Reflex testing for ANA titer and pattern is performed for specimens with a positive ANA IFA result. The ANA screen has high sensitivity (97%) but limited specificity (34%) for SLE; anti‑dsDNA and anti‑Sm antibodies are less sensitive but more specific and both are included in SLE classification criteria. Anti‑chromatin antibodies have relatively high sensitivity and specificity. Although SSA, SSB, and RNP antibodies are more associated with Sjögren syndrome or mixed connective tissue disease, they are also frequently present in SLE and may be diagnostically helpful. Results should be interpreted in clinical context including patient history and physical findings.

Not provided.
Limitations
ANA testing exhibits limited specificity despite high sensitivity; presence of autoantibodies may overlap with other connective tissue diseases; interpretation of staining patterns and titers requires clinical context. Reflex testing incurs additional charges when positive.
