Cell Surface (Epithelial) Antibodies, IgG by IIF

Use
This test assesses and monitors cell surface (CS) antibodies, also known as intercellular substance (ICS) antibodies, in patients with IgG pemphigus variants. It is useful for cases where normal IgG desmoglein 1 and IgG desmoglein 3 antibody levels are present. The testing is recommended to be correlated with concurrent Direct Immunofluorescence on a tissue biopsy, as CS/ICS antibodies by indirect immunofluorescence are commonly positive in IgG pemphigus variants, including pemphigus foliaceus and pemphigus vulgaris.
Special Instructions
Testing should initially be correlated with concurrent Direct Immunofluorescence, Tissue Biopsy (Cutaneous, Mucosal, Epithelial) (0092572). The Serum Pemphigus Antibody Panel, IgG (0090650) has increased diagnostic sensitivity and specificity for pemphigus. For comprehensive testing, consider ordering the Immunobullous Disease Antibody Panel (3001409).
Limitations
This test does not exclude the diagnosis of pemphigus vulgaris, pemphigus foliaceus, or another IgG pemphigus variant if negative. Indirect immunofluorescence testing results need clinical correlation, including direct immunofluorescence findings and treatment status. Cell surface reactivity may be observed transiently and/or nonspecifically in normal individuals or in cases of drug reactions, infections, and other mucocutaneous diseases.

Transfer 2 mL serum to an ARUP standard transport tube.
Storage Instructions
Store refrigerated unless shipping promptly (within 2 hours). Transport at ambient temperature to arrive within 7 days. If 7-14 days until received in laboratory, store and ship refrigerated. If greater than 14 days, serum specimens must be stored and shipped frozen.
