Use
This test is most useful if severe FXIII deficiency is suspected, with less than 1% of normal activity. It is designed to distinguish between FXIII deficiency and a FXIII inhibitor. Abnormal results should be confirmed with quantitative testing using the Factor XIII Activity test. The test is especially relevant because severe deficiency may be inherited or acquired due to a factor XIII antibody. The reflex to a 1:1 mix helps further distinguish between FXIII deficiency and an inhibitor if clot lysis occurs in the initial test.
Special Instructions
This is a qualitative screening test where clot lysis will occur only in specimens with severe factor XIII deficiency, defined as less than 1 percent of normal activity. If the initial test shows clot lysis, the Factor XIII 1:1 Mix will be added. The test is performed daily from Sunday to Saturday, with results reported in 2-3 days.
Limitations
The screening test may give false-positive results due to heparin therapy or contamination, decreased or abnormal fibrinogen, increased fibrinolysis, fibrinolytic drugs, or factors affecting clot stability. It does not identify mild or moderate deficiencies, treated deficient patients, or patients with weak inhibitors that do not significantly decrease factor XIII activity. Further quantitative testing is recommended for confirmation of severe deficiency or inhibitor presence.
