17‑hydroxyprogesterone is elevated in patients with congenital adrenal hyperplasia (CAH), a group of autosomal recessive diseases characterized by cortisol deficiency and elevated ACTH. It is useful for monitoring cortisol replacement therapy and evaluating infertility and adrenal or ovarian neoplasms.
