Deamidated Gliadin Peptide (DGP) Antibody, IgA

Use
A positive deamidated gliadin (DGP) IgA antibody result is associated with celiac disease but is not to be used as an initial screening test due to its low specificity and only occasional positivity in celiac disease patients who are negative for tissue transglutaminase (tTG) IgA antibody. It may be used to evaluate for suspected CD in individuals with negative or weak-positive results for tTG IgA and EMA IgA.
Special Instructions
Not recommended as an initial test to evaluate for suspected celiac disease (CD). The preferred test to screen for CD is Celiac Disease Reflexive Cascade, Serum (3016817). All serologic tests for CD should be performed while the patient is on a gluten-containing diet. Upon initiation of a gluten-free diet, antibody titers decline in treatment-responsive patients.
Limitations
Deamidated gliadin peptide (DGP) IgA has low specificity and limited role in identifying undetected CD. In individuals who produce sufficient total IgA, the most sensitive serologic test for CD diagnosis is tissue transglutaminase (tTG) IgA. Individuals who are IgA deficient should have tTG IgG and DGP IgG antibody testing. Reflexive panel testing is preferred to screen for CD.
