Deamidated Gliadin Peptide (DGP) Antibody, IgG

Use
This test is used to evaluate for suspected celiac disease (CD) in individuals who are deficient in IgA. It measures the level of deamidated gliadin peptide (DGP) antibodies of the IgG isotype, which may indicate celiac disease when the results are positive. However, due to the lower accuracy of these markers, particularly in patients without IgA deficiency, a small intestinal biopsy is required to establish a definitive diagnosis.
Special Instructions
For patients who produce enough total IgA, tissue transglutaminase (tTG) IgA is the most sensitive and specific serologic test for celiac disease diagnosis. In IgA-deficient individuals, tTG IgG and DGP IgG antibody testing is suggested. The test should be performed while the patient is on a gluten-containing diet, as antibody levels decrease upon initiation of a gluten-free diet in responsive patients.
Limitations
The test has lower accuracy in patients without IgA deficiency. If serology results are negative, but the suspicion for celiac disease remains high, an intestinal biopsy may still be needed to confirm the diagnosis.
